The referral system is a vital part of any medical treatment system that focuses on supplying women with needed healthcare between the course of their pregnancy and delivery. All the referred obstetric cases were analyzed for the cause of referral, place of referral and outcome for the period of 6 months from October 2021 to March 2022. In our study we found total 486 obstetric patients were referred. Maximum referred patients were in the age group of 20-30 years and 61% referrals were from Dehradun district 12.96% of cases were delayed referrals. According to the findings, illiteracy and ignorance are important contributors to poor pregnancy outcomes. The need of the hour is to train primary and secondary health care workers in essential and emergency obstetric care in order to avoid unnecessary referrals and reduce the burden on tertiary care hospitals, as well as to reduce the frequency of delayed referrals in order to improve maternal and fetal outcomes.
